​This report is derived from the Indiana State Department of Health's Ambulatory Surgical Centers' Indiana Utilization Report for selected year, annual database and is organized alphabetically by free-standing ambulatory surgery center name.   For each center that reported to ISDH, the report lists the selected years volume for their most frequent CPT codes performed.
