The Hospital Quality Improvement Collaborative (HQIC) is a voluntary program funded by the Center for Medicare & Medicaid Services (CMS) to support eligible hospitals in rural, critical access, and urban settings that provide care for vulnerable populations with the goal to reduce harm, improve care transitions, and advance engagement and equity. Areas of focus include but are not limited to; readmissions, sepsis mortality, medication safety, and device-associated infections. Essential strategies that are woven throughout are patient, family, and caregiver engagement, health equity, and public health emergencies.
Technical assistance includes coaching and other forms of technical assistance including data collection and analysis. In the spirit of quality improvement, coaching is available at the local, regional, and state level. This includes monthly convening of all participating organizations, cohort group coaching, and individual site visits to align with local priorities.
